Middle Ear Infection

A middle ear infection, also known as otitis media, is a common childhood illness. Nearly half of all children experience a middle ear infection by the age of 10, with preschoolers being especially affected. This inflammation impacts the mucous membranes in the air-filled tympanic cavity behind the eardrum and often accompanies severe ear pain. In about half of the cases, it also involves fever. Hearing impairments, stomach pain, and vomiting may also occur.

In babies, it is more challenging to recognize a middle ear infection. Signs to watch for include fever of unknown origin, difficulty drinking, excessive crying, or unusual restlessness and irritability. The inflammation often follows a cold or flu, where swelling of the mucous membranes impedes the ventilation of the middle ear, leading to mucus buildup.

The diagnosis of a middle ear infection is usually made by observing the highly red, bulging, or retracted eardrum with an otoscope. A tear in the eardrum due to the inflammation can occur, resulting in sudden relief of symptoms. Treatment includes decongestant medications or nasal drops to promote the drainage of secretions.

Antibiotic treatment is only necessary for bacterial infections and should be administered for at least a week to prevent recurrences. Paracetamol or ibuprofen can be given as syrup or suppositories to relieve pain and reduce fever. A proven home remedy is the onion poultice, where a chopped onion is placed on the affected ear.

It is important to treat middle ear infections promptly to prevent the spread of the infection to the mastoid bone behind the ear. Adenoids in the nasopharynx can cause recurrent ear infections and may need surgical removal. A vaccine against pneumococci, a common cause of middle ear infections, is available.
